Certificates of Attendance are available for $5.00 for educators and other professionals. Please complete the following steps to ensure that you receive your PLU/CEU credits for attending the 2012 “Embracing Exceptionalities” Conference.
- If required by your administrator, complete the Prior Approval Form and have it signed by your school superintendent or designee prior to the conference. (This form is for your records, you do not have to turn it in to conference personnel.)
- If you pre-registered for a Certificate of Attendance, there will be a Session Attendance Confirmation Form included in the materials you receive when you check in on the first morning. This form should be completed during the conference.
- Attend sessions at the conference and receive a verification sticker to place on your Session Attendance Confirmation Form from the facilitator in EACH session that you attend as you depart the session.
- Before leaving the conference, turn in your completed Confirmation Form to the Professional Learning Desk at the conference.
A Certificate of Attendance with the number of direct contact hours you received (based on your confirmation form) will be mailed to the address listed on your Prior Approval Form within 2 weeks of the end of the conference. There are 10.5 direct contact hours available for the “Embracing Exceptionalities” Conference.
Following the conference, it is your responsibility to submit the above information to the appropriate personnel in your district.
